Fen�meno de retardo en el m�sculo esquel�tico. Un estudio experimental

RESUMEN
La transferencia de tejidos es un procedimiento ampliamente usado en cirug�a pl�stica. La disponibilidad de sitios donadores particularmente de gran extensi�n est� limitada por la arquitectura vascular. La aplicaci�n del fen�meno de retardo y los conceptos actuales sobre revascularizaci�n y angiog�nesis permiten al cirujano reconstructivo manufacturar nuevos colgajos de gran refinamiento y sofisticaci�n. Desarrollamos un modelo experimental de transportador vascular para prefabricar un gran colgajo musculocut�neo en la rata. Se compar� la capacidad de revascularizar un colgajo compuesto de m�sculo trapecio a trav�s de un retardo de m�sculo esquel�tico del m�sculo dorsal ancho como ped�culo m�sculo-vascular con un control no retardado. El ped�culo m�sculo-vascular exhibi� la mejor revascularizaci�n del colgajo compuesto prefabricado con un 90% ± 10% de supervivencia del colgajo (p‹0.001), mientras que el no retardado mostr� necrosis total en el 85.8% de los controles. La revisi�n histol�gica del modelo mostr� evidencia visual de revascularizaci�n y formaci�n de un ped�culo angiog�nico entre el m�sculo dorsal y el trapecio que proporciona supervivencia a este �ltimo. Se establece la posibilidad confiable de elaborar colgajos m�sculo-musculares compuestos por medio del retardo y el desarrollo de un ped�culo angiog�nico.
